PADI Open Water Diver certification

A student diver should present the PADI Universal Referral form for Open Water Diver Certification (PDF) to a certified instructor. It states that the diver took initial training from another diver training organization. The form directs the instructor of diving to complete the student’s open-water dives and sign a declaration. This statement should be sent to the original instructor so that they can certify the diver.

While you can use the Universal referral form to get the certification, PADI requires that you complete the course within twelve months of your last training. Usually, this is a two or three-day course. During this time, you must be able to dive with your buddy to a maximum depth of 60 feet/18 meters.

Medical statement

A Medical Statement for universal Referral Form is required if you are interested in becoming a certified scuba diver. This is an important document that will ensure you're as healthy as possible before you go diving. It is intended to ensure safety underwater. Please answer all questions honestly.


First, ensure that your doctor signs the form. This is mandatory when you begin your training as an scuba diver. A new medical release is required if you have a medical condition. Last but not least, make sure you have a photocopy to your certification card.

Logbook

A medical statement is required before a diver can open-water dive. Generally, this will be done before a certification course begins. The medical release must be signed by a physician. You will need a new release if your medical situation changes. A medical release is available through PADI. This medical release is equivalent in size to the PADI Liability Release/Assumption and Risk Form - General Training, or the EU version.

The universal referral is different from that used in a diving logbook. It will list the different open water dives and knowledge development sections that the student completed. It also contains a signed statement from the instructor confirming that the diver has taken the required dives. The student should then return this form to the original instructor who will be able to certify them as certified divers.

Course completion requirements

The Universal Referral Program covers most well-respected scuba certification bodies. The Universal Referral Program means that a diver may take the theory or pool portion of an open water course with their local dive center and then do the open water check out dives with another agency. This is done with the cooperation of a dive center with a universal referral program such as SSI, NAUI, NASDS, YMCA, or other organizations.

PADI Open Water Diver courses require that the student passes the Knowledge Development and Open Water Dives sections. Before diving, the student needs to sign a medical release. To be certified, the student diver must take a photo. Digital images are acceptable.
